Our homes are categorized in two ways to help families find the perfect fit. After we perform a nursing assessment that measures your loved one’s physical mobility and cognitive level, we will help place him or her in either assisted living or skilled care.
Upon evaluation and admittance, a personalized care plan is developed specifically to address your loved one’s needs. Our goal is to ensure a natural transition to a senior care environment that is most like our resident’s own previous home.
Our Services
Assisted Living
Assisted Living is based on a loved one’s needs for daily care. This includes meal preparation, bathing, dressing, medication maintenance and encouraged socialization through daily activities with others. Mobility with a walker or self-propelled wheelchair (scooting with feet) is common in our assisted living homes.
Skilled Care
Skilled Care is based on a loved one’s needs for advanced help when they are no longer cognitively aware/communicative, have a more advanced medication/care directive and/or have become non-ambulatory.
Well-Being Services
We are dedicated to providing specialized care to meet the needs of each resident individually. Our caregiver-to-resident ratio is no higher than 1-6. Because care is provided in a home environment rather than a facility or institution, our professional caregivers are in close proximity and are immediately available to assist residents at all times, 24 hours a day.
- We provide a central living and dining area to encourage socialization and reduce isolation.
- We encourage resident independence, freedom and familial visitations/excursions.
- Meal preparation following dietitian guidelines (specialized diets included)
- Medication administration
- Personal assistance bathing, dressing, toileting
- Housekeeping
- Linen Service
Health Management Services
- We work directly with your physician and insurance provider to help you facilitate doctor appointments and physical therapy appointments on/off site.
- Our Nurse rotates through each home on a standard schedule and can be paid directly through your health insurance provider. This helps with routine health concerns.
- Senior Care contracts with an on-call Physician to address emerging/emergency health concerns.
